The Nurse Practitioner (NP) Home-Based Primary Care (HBPC) Nurse Practitioner is accountable for the management and evaluation of a panel of patients under his/her care in the veteran's home setting or via clinical video telehealth. The HBPC NP provides comprehensive preventative and wellness care, as well as the spectrum of acute, episodic, chronic, and end-of-life care to Veterans assigned to his/her HBPC Patient Aligned Care Team (PACT) Team.
The NP HBPC uses the nursing process and evidence-based practice to work collaboratively with the Team and the family/caregiver, internal and community-based services, and disciplines involved in providing care to the patient, in developing the patient-driven holistic care plan for life. The NP HBPC provides diagnosis and treatment and provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
The NP HBPC is prepared in advanced practice across the health continuum. This role includes participating in and use of research, development, and implementation of health policy, leadership, education, case management, and consultation.
The HBPC Nurse Practitioner utilizes preventative clinical guidelines and appropriate standards of care in collaboration with the Home-Based Primary Care Medical Director in providing quality care to patients with complex chronic and acute on chronic health conditions.The NP HBPC:

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to wear protective clothing in isolation situations or operative/invasive procedures.
