Role Scope

The Referral Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal. They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals. The referral coordinator is an integral part of the care team, working as a liaison between the patient, providers, utilization management and various departments to ensure timely access to specialty care.

  • Receive, review, and process referral requests from healthcare providers.
  • Coordinate with specialists, hospitals, and other healthcare providers to schedule participant appointments.
  • Ensure all referral information is accurate, complete, and compliant with healthcare regulations.
  • Serve as a primary point of contact for participants, providers and speciality network
  • Provide participants with information on referral processes, appointment details, and collaborate with care team on any required pre-appointment preparations.
  • Maintain effective communication with healthcare providers to ensure timely updates on referral statuses.
  • Verify and update participant records in the electronic health record system.
  • Prepare and submit reports on referral activities and outcomes.
  • Assist participants with understanding their PACE benefits related to referrals.
  • Participate in provider relations or joint operations meetings, as requested.
  • Performs related duties as assigned.

Required Qualifications

  • High school diploma or GED required.
  • Minimum of 2 years of experience.
  • Strong organizational and multitasking abilities.
  • Excellent communication and interpersonal skills.
  • Proficiency in using EHR systems and other healthcare-related software.
  • Knowledge of medical terminology, insurance processes, and healthcare regulations.
  • Reliable and timely transportation to and from primary work location, and other locations to fulfill responsibilities listed. Reimbursement will be provided according to company mileage policy.

Preferred Qualifications

  • Associate’s or Bachelor’s degree in healthcare administration, medical assisting, or a related field preferred.
  • Medical Assistant certificate.
  • Bilingual: Spanish/English preferred.
  • A state issued driver’s license, personal transportation, and auto insurance as required by law.

Compensation

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $22.00 -$25.00 and is bonus eligible. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
